HACKENSACK MEDICAL CENTER BENEFIT PLANS

Advantage Health Plan - An open access Point of Service Plan with three tiers of benefits. There are over 780 participating physicians in the Inner Circle Network and over 100 NJ hospitals and over 24,000 providers participating in the QualCare Network.
 
Delta Dental of New Jersey Dental Plan - There are four different tiers in the Delta Dental Plan. Most basic dental work such as fillings, extractions, root canals, periodontal, crowns and bridgework are covered at 80%.
 
Prescription Coverage - The Plaza Retail Pharmacy on-site at Hackensack Medical Center offers a 90 day supply of medication. It's the only retail pharmacy that allows you to get a three month supply. You may also purchase your prescription drugs through Medco mail order or 60,000 participating chain and independent pharmacies.
 
Vision Plan - Offered through Vision Service Plan, benefit-eligible employees can receive a yearly eye exam for a co-pay of $15 at participating optometrist or ophthalmologists in network, a $30 co-pay toward the frames of your choice up to $130 plus 20% off any out-of-pocket costs, or a $120 allowance toward the lenses and the fitting and evaluation exam for contact lens.
 
Basic Group Life and Accidental Death & Dismemberment Insurance - All full-time benefit eligible employees are entitled to 1x their base annual salary rounded to the higher thousand at no cost to you. Accidental Death & Dismemberment also provided at 1x your base annual salary.
 
Paid Time Off:
Vacation Plan - Full and Part-Time benefit eligible employees are eligible. Newly hired employees begin to accrue vacation time the first pay of the month following three months (3) of service. Full-time employees' entitlements are as follows:
 
  2 Week Category 3 Week Category
After 1 year: 80 hours 120 hours
After 3 years: 120 hours 160 hours
After 10 years: Add 8 hours per year  
Maximum: 160 hours 200 hours
 
Part-time employees receive one half of the full-time allotment.
 
Sick Time - Upon completion of three (3) months of service, you will accrue 8 sick time hours per month up to a maximum of 960 hours if you are a full-time employee. Part-time employees accrue 4 hours per month up to a maximum of 480 hours.
 
Holiday Time - Employees receive 6 paid holidays. They are New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
Optional Holidays - In addition, employees receive four (4) paid optional holidays to use as personal days. One day is earned each quarter and is prorated the first year of employment based on hire date.
 
403(b) Retirement Savings Plan - Under the Retirement Savings Plan, you will receive an automatic HUMC contribution of 2% of your plan compensation each year, provided you work 1,000 hours in the plan year and are employed on December 31 of that year. Employees must work a minimum of 20 hours per week. In addition, you will be eligible to receive an HUMC matching contribution on a portion of your salary deferral contributions you make to the Retirement Savings Plan. In your first 5 years of employment, HUMC will match 50% of your 403(b) deferrals up to 3% of your pay each pay period. The matching contributions will increase as your service with HUMC increases. Please note that compensation is limited to $245,000 in calendar year 2011 and is subject to change annually. Plan features include:
 
  • Voluntary pre-tax salary deferral contributions up to 100% of earnings up to permissible limits ($16,500 for 2011)
  • You are always 100% vested in your salary deferral contributions. You will be 100% vested in the automatic 2% HUMC-paid contribution and matching contributions after completing 3 years of service (at least 1,000 hours per year).
  • Employees turning age 50 or older by the end of the calendar year may also make catch-up contributions up to permissible IRS limits ($5,500 for 2011).
  • Employees who have at least 15 years of completed benefits-eligible service with HUMC may be eligible for an additional $3,000 in catch-up contributions.
  • You choose how to invest all contributions made to your Retirement Savings Plan Account to fit your savings and investment goals.
  • The entire balance of your account will be paid to your spouse or other designated beneficiary if you die.
 
Temporary Disability Insurance - Pays up to 2/3 of your gross salary up to a maximum of $559 per week for 26 weeks when you cannot work due to your temporary disability.
 
Workers' Compensation - Provides you with protection for loss of income and medical expenses when on-the-job accidents or injuries occur.
 
VOLUNTARY PLANS:
 
Critical Illness Insurance - Pays a lump sum benefit upon initial diagnosis of a covered critical illness, including Heart Attack, Stroke, Cancer, Occupational HIV and others. Benefits payable for multiple events and plan includes a Wellness Benefit that covers several screening tests.

  • Coverage is convertible to an individual plan at the same or similar benefits and rates if you leave HUMC
 
Accident Insurance - On-the-job or off-the-job coverage for accidental injuries with benefits paid directly to employee (unless assigned). The benefit includes an Outpatient Physicians Treatment Benefit for visits to any doctor outside of a hospital, for any reason.
 
Short Term Disability Insurance - Provides additional income protection to supplement the current NJ State Disability benefits up to a maximum of $2,500 per week (carving out the State weekly TDB benefit).
 
Long Term Disability Insurance - Protects your salary up to 60% up to age 65 for active employees who have exhausted their short term disability plan and are disabled due to an illness or on a leave of absence.
 
Health Care Flexible Spending Account - Helps you save up to $5,000 tax free dollars to pay for eligible health care expenses that are not covered under your health plan such as co-payments, deductibles, prescription drugs, dental work, vision care and certain over-the-counter items.
 
Dependent Care - Also a $5,000 tax savings for employees with dependents in day care. For your convenience, Hackensack Medical Center also has an on-site daycare center at the Sarkis and Siran Gabriellian Child Care Learning Center.
 
Tuition Assistance Program - The tuition program provides financial assistance for hospital work-related courses. Reimbursement is for tuition only up to the current IRS limits. For full-time employees, the limits are $4,250 for undergraduate and $5,250 for graduate studies. Part-time employees have limits of $2,125 for undergraduate and $2,625 for graduate studies.
 
529 College Savings Program - A college savings program that allows you to save for your children's college expenses offered through the convenience of payroll deduction. Distributions for qualified education expenses are tax free.
 
Met Life Auto & Home Insurance - Provides employees with auto and home insurance at special group rates and discounts up to 10%.
 
Met Life Pet Insurance - Insurance to help you handle the ever-increasing costs of caring for your pet when they are ill.
 
NJ Manufacturers Home & Auto Insurance - Offers competitive rates and discounts provided each year on your policy renewal date.
 
Computer Purchasing Program - Allows employees to purchase new computers regardless of credit status with the convenience of payroll deduction over a 12 month period. Certain eligibility requirements must be satisfied to enroll.
 
Vacation Purchasing Program - Genesis Travel Benefits offers employees discounted rates on-line deals on cruise and vacation packages throughout Europe, Florida, and the Caribbean. This program allows you to pay by credit card or finance by payroll deduction.
 
LegalEase Insurance Services Plan - For a small monthly fee, employees have the option to purchase a wide variety of legal services such as will and estate planning, real estate closings, family law, financial protection, litigation and trial defense.
 
Greater Alliance Federal Credit Union - Provides many savings and loan programs as well as holiday and vacation clubs.
 
Employee Assistance Program - All employees and members of household may utilize this free benefit. Professional counseling and referral services are available to help you with a wide range of personal concerns.
